This chest is referred to as IDOC loot, it's from a player house that has collapsed (IDOC = In Danger Of Collapse).
When a player house collapses there is a 50% chance of any item in it being random loot on any monster spawn...
Most of the single image edits (treehack, traps, runes etc) aren't hard it's just a matter of using the right tools to extract and find the image file and replacing it.
The anims are quite a bit more complicated as you have to extract them and edit each image (can be hundreds of them).

and fair play...if you've done any UO edittng you'll know it's a lot of effort shrinking every image that makes up the anims (like 450ish for each drag model) and editing the anim meta data so they sit right.

2: yes - 3 accounts is the limit and shouldn't be a problem creating 3rd, possibly an issue with account name?
3: no-cut refers to not being able to cut bodies up after death, so no stat-loss for PK's...

I guess it's just easy to dismiss the odd connection issue due to the fact the that UOF's host had been having more than it's fair share of problems over a few months or more.
I think a less determined person would just accept that packet loss leading to the odd random client disconnect is...
probably you already did this, but if not the first suggestion would be check if a new firmware update is available! :)
But bear in mind the UOF host has still been having some issues by the sounds of it.
