Well I am not a noob per se, just havent played in years.

Anyways, can someone remind what the difference between skill levels and "real" skill levels are?

Real skill is what your character has actually learnt where as uncheck it shows that plus modifiers. E.g. accuracy on a bow will increase your archery by 5-20 points. Also being human your a "jack of all trades", so get some skill is skills you have 0 learnt points in meaning you stint necessarily fail :)

Or something like that :p

Yeah for levelling you want to go off your your real, or remove items you're wearing that increase your skill. Lower level = faster gains :)

I believe singe kills will look at over all skill but I'm sure some will only look at real values... remeber reading about it but can't remember where. Just look at real skill and you won't go wrong :)

Huh. I always went by real skill when leveling up.

You always want to go by the "show real skill" because as you level a particular skill, you will start to lose out on the "extra points" awarded. For example, my tamer has 90 veterinary skill as a "real" skill, but has an additional 1.6 added, so, technically, I am at 91.6 as a "soft" skill. However, as a lower real veterinary skill, I might get an extra 5 points, as a cushion. Where I am going with this, as you level up skills, you will lose out on the "soft" or cushion skills (I don't know if there is an actual word that describes it, so I am improvising. lol). Eventually the "soft" skill will vanish. So, for taming, for example, when you reach 99 (don't hold me, it has been a while), but you do not get any more "soft" skill. When raising your skills, you have to remember that your "real skill" cap is 700, unless you go the route of using power-scrolls (only select skills allow you to go higher, which can be found on the wiki page), then you can go to 720. Hopefully, this kind of helps you? I tried to explain it the best I could.
